Become a Member of Your Local Family Care Council

 Make a difference in the lives of Floridians with developmental disabilities. As a member, you help shape services, advocate for families, and ensure every voice is heard. If you’re passionate about advocacy and community involvement, see below for eligibility and how to join the FCCs!

Each local Family Care Council has 10–15 members, as required by Florida law.(Section 393.502(2), Florida Statute).


  • At least 3 members must be individuals who are currently receiving or waiting to receive services from the Agency for Persons with Disabilities (APD).
     
  • At least 1 of those individuals must have received APD services within the past 4 years.
     
  • The remaining members may be parents, grandparents, guardians, or siblings of individuals with developmental disabilities who qualify for APD services.
     
  • If a grandparent serves as a council member, the parent or legal guardian of the individual must give consent, which must be reported to APD. 


Each applicant must be approved by a majority vote of the local Family Care Council, as required by Florida law.


To support the council’s review, applicants are asked to complete the FCC Membership Interest Form. This form includes the applicant’s eligibility information, a brief biography, and their interest in serving on the council.


Once the completed form is submitted, the council will review the application and vote on the appointment.


Once approved, the chairperson will provide a letter of recommendation. This letter must be uploaded to your digital application. 


Eligible candidates must submit the State of Florida Appointments Questionnaire through the Governor’s Appointments Office online portal.


To ensure your application packet is complete, please follow the guidance below:


  1. Visit the Governor’s Appointments Office portal:
    https://eogforms.eog.myflorida.com/pages/SeatApplication.aspx
     
  2. Download and complete the Gubernatorial Appointments Questionnaire: https://flgov.com/eog/sites/default/files/documents/Gubernatorial-Appointments-Questionnaire-CACH.pdf
     
  3. Upload the completed questionnaire in Step 2 of the portal.
     
  4. Complete all required steps in the application.
     
  5. Upload the Letter of Recommendation in Step 5.
     
  6. Be sure to click “Submit Completed Application” in Step 7 to finalize your submission.


After you submit your application, you will receive a confirmation email from the Governor’s Appointments Office. If you do not see the email in your inbox, please check your junk or spam folder for a message from Appointments@eog.myflorida.com.


The email subject line will read:
“Successful Application :: EOG:XXXXXXXXX”

The number shown is your applications confirmation number.


Applicants are encouraged to forward this email to their FCC Chairperson and APD Liaison. APD works with the Governor’s Appointments Office to gather additional information and support pending FCC applicants.
